Xray Cloud uses entities (Jira issues) which are managed and hosted direclty by Jira Cloud.

Besides this, Xray Cloud uses its own database and supporting infrastructure which are hosted in proper, secure cloud vendors. 

This means that Xray Cloud and Jira Cloud need to communicate between each other using proper secure APIs (e.g for obtaining issues, to be notified upon changes on some issues). 

However, sometimes due to connection/networking issues or simply because you disabled Xray for some time, Xray Cloud database may be out-of-synch and you may need to enforce a synchronization, which we call "project re-indexing". 

Whenever using Xray, if out-of-synch issues are detected then a warning message will be shown to the user so he can be aware of it and take an action in order to resolve it, by doing a project re-index.

The way apps are deployed and run om Cloud follows a different approach from Jira Server, due to the way Jira Cloud architecture was designed by Atlassian.

On Jira Server, apps such as Xray run directly in the web container of Jira; on the other hand, the Cloud apps must run outside of Jira Cloud infrastructure and use secure APIs in order to interact with Jira.
